2.3 REPORTING INJURIES AND ACCIDENTS

a. Each work injury or illness is to be reported IMMEDIATLEY to the first aid attendant and to your Job Supervisor to ensure suitable first aid or medical attention. You must also notify you Master Plumbers Apprentices Limited as soon as possible.
b. All accidents involving damage to equipment and materials, including motor vehicle accidents, must be reported to your Field Officer and Job Supervisor at once so that investigation mat be immediately actioned to establish the cause of the accident and determine that a safe condition prevails.
c. Accidents will be investigated using Master Plumbers Apprentices Limited's Accident/Incident report form.

The main reasons for investigating accidents are:

To prevent recurrence
To identify the causes.
The accident investigation is NOT to appropriate blame.


2.4 REPORTING UNSAFE OR HAZARDOUS CONDITIONS
Report unsafe of hazardous conditions to your immediate supervisor so that corrective action can be taken to prevent possible accidents. Discuss identified hazards with your work area Health & Safety Representative supervisor.

Advise your Field Officer of any hazardous situations that constitute a risk to your health & safety or the health & safety of other workers that have not been corrected.
